Notice of Information Practices
April 15, 2003

Each time you participate in a work-site health screening, Health One makes a record of your visit. This recording may contain your health history, current symptoms, examination and test results, treatment, and a recommendation for follow-up. This record could also simply be a sign in sheet. This information serves as a:
1. means to provide you with your health information
2. means by which you or a third-party payer can verit& that you received the services billed for
3. a source of information for public health officials charged with improving the health of the regions they serve, for example positive TB skin tests or positive anthrax tests.


Understanding what is in your health records and how your health information is used help you to:
1. ensure its accuracy and completeness
2. understand who, what, why, and how others may access your health information
3. make informed decisions about authorizing disclosure to others
4. better understand the health information rights detailed below
5. decide if you want to participate in a work-site public health screening

Your rights under the Federal Privacy Standard

Although your health data are the physical property of the Health One, you have certain rights with regard to the information contained therein.
1. Health One only uses health protected health information to; provide you with results, create summary reports for your employer using aggregate data, inform you of up-coming Health One events, and perform health care operations. ‘Health care operations” consist of activities that are necessary to carry out the operations of the provider, such as quality assurance and peer review. The right to request restriction does not extend to uses or disclosures permitted or required under §~ 164.502(a)(2)(I)(disclosures to you), 164.510(a)(for facility directories, but note that you have the right to object to such uses), or 164.5 12 (uses and disclosures not requiring a consent or an authorization.) The latter uses and disclosures include, for example, those required by law, like mandatory communicable disease reporting. In those cases, you do not have the right to request restriction. You may also ask us to communicate with you by alternate means, if the method of communication is reasonable, we must grant the alternative communication request.
2. You have the right to obtain a copy of this notice of information practices.

Our Responsibilities under the federal privacy standard
1. maintain the privacy of your health information, including implementing reasonable and appropriate physical, administrative, and technical safeguards to protect the information
2. provide you with this notice as to our legal duties and privacy practices with respect to individually identifiable health information we collect and maintain about you
3. abide by the terms of this notice
4. train our personnel concerning privacy and confidentiality
5. discipline those who breach privacy/confidentiality or our policies with regard thereto
6. mitigate (lessen the harm of) any breach of privacy/confidentiality

We will not disclose your health information without your consent or authorization, except as described in this notice or otherwise required by law. If you have questions and/or would like additional information, you may ask to contact the privacy officer or director of health information. We reserve the right to charge a reasonable, cost-based fee for making copies.
